The Problem

During my time at BrainStation, I was tasked to identify a key problem patients are having with the current healthcare system and designing a digital solution for it.
MyHealthHub is a government-led initiative focused on improving patients experience with the health care system.
Secondary Research
My research started by gaining an understanding of the current challenges patients face while managing their health through existing healthcare services and sources of information. I discovered that people with multiple chronic illnesses struggle to manage their care due to miscommunication and poor coordination among the different health professionals involved in their treatment.

User Interviews
I interviewed 4 people who were managing two or more chronic conditions, either on their own or with support from a caregiver. Participants described gaps in communication between providers, limited access to their medical information and administrative hurdles. The theme I focused on:

She meticulously tracks her medical records because she’s frustrated by the lack of coordination between her healthcare providers. She wants a convenient and accessible way to manage her health.

Design
I started the design process by creating a task flow for users to create a profile with their medical information. Users will only need to share these details in the profile set up so they can navigate the app without having to repeat information.

Testing + Improvements
I conducted a usability test with 3 participants and received valued insight on improvements to the overall experience of the app. Here are the changes I made based on their feedback:
